The 2027 Medicare Advantage (MA) landscape is out, and the headline is exits. At least eleven insurers that sold individual MA plans in 2026 will not offer them in 2027, up from seven that left for 2026 [1][2]. Seven of the eleven are owned in whole or in part by health systems [1].
Coverage of this tends to focus on the member: who gets a letter, which plan to pick before December 7. This note looks at the other side. For the organization, an exit, a county pullback or a portfolio restructuring is a transition, and most of the work starts after the announcement.
Why now
- Full exits. Wellmark BCBS is leaving individual MA across 98 Iowa and 43 South Dakota counties; BCBS North Dakota across all 53 counties in the state; Christus Health Plan across 36 Texas and 8 New Mexico counties [1].
- Exits inside larger shutdowns. Providence Health Plan's MA exit is part of a wider shutdown of the plan, which is also leaving the commercial and most Medicaid markets in 2027 [1][2]. Memorial Hermann Health Plan confirmed in July that it is also closing its commercial business [1].
- County pullbacks. Centene is leaving 344 counties for 2027, according to Stephens [3]. Humana has told investors its 2027 exits affect about 600,000 members [4]. In Minnesota, HealthPartners is discontinuing three MA plans and leaving seven counties, affecting about 24,500 members [5].
- Restructuring without exiting. Blue Cross and Blue Shield of Minnesota is discontinuing its 2026 MA plans to rebuild its Medicare portfolio for 2027, and says more than 80% of affected members will have a new Blue Cross option [5].
- Fewer places to land. 181 counties will have no MA plan in 2027, up from 67 this year, and CMS has for the first time set hard enrollment caps on 15 regional insurers in 11 states [2].
Not every large carrier is retreating. UnitedHealthcare says its individual MA footprint grows from 2,787 to 2,791 counties in 2027, while exiting Wyoming [6]. The common pattern is margin over membership: insurers are giving up enrollment to repair margins for a second year in a row [3]. The MA County Viability Check runs that same test on a single county.
What an exit sets in motion inside the plan
The regulatory calendar is fixed. A plan that does not renew must notify CMS in writing by the first Monday in June of the year the contract ends, and notify each enrollee by mail at least 90 calendar days before the nonrenewal takes effect [7]. That notice must come with a CMS-approved description of alternative plans, or the plan must place outbound calls to all affected enrollees [7]. Nothing about the nonrenewal can be made public before CMS notifies the plan [8]. A plan that does not renew can also be barred from a new contract or a service area expansion for two years [7].
The calendar ends on December 31. The work does not:
- Claims runout. Claims for 2026 dates of service keep arriving and being adjusted well into 2027, against a contract that no longer exists and, often, a shrinking team.
- Provider contracts. Network agreements tied to the exiting product have to be terminated or amended, and settlement terms (risk arrangements, withholds, disputes) closed out.
- Risk adjustment and encounters. Final submissions for the last payment years still drive revenue after members are gone. If the submission pipeline depends on people or vendors being released, revenue goes with them.
- Records. Member, claims and submission data have to stay retrievable and auditable after the systems that produced them are retired.
The other side: absorbing members
Exits move members, they don't remove them. Plans that stay in a county, or enter it, receive members on January 1 with no history in their own systems: no claims, no care management notes, no risk-adjustment record. Network adequacy, identity matching and onboarding have to work from day one, and in capped markets, growth itself becomes a compliance question [2].
What any plan in transition should check
Exiting, restructuring and absorbing are the same problem from different angles: systems, data, vendors and people have to converge or wind down without losing control. The Health Plan Transition Checklist covers that in 20 questions across five areas. Applied to a market exit:
- Systems & platforms. Which systems serve only the exiting product, who owns their wind-down, and when they can actually be retired after runout.
- Data & reporting. One member identity across terminated and continuing contracts; claims paid reconciled against expected through runout; final risk-adjustment submissions traceable end to end.
- Identity & access. Access removed for staff and vendors tied to the exiting business, while audit access is preserved.
- Vendors & regulators. Data return and retention terms in every Business Associate Agreement; one owner for the claims processor relationship. Vendor risk is not theoretical here: CMS suspended new enrollment into Aspirus Health Plan in February after its claims processor, UCare, was taken over by Minnesota regulators [1].
- People & sites. Workforce reductions sequenced against runout, not against the contract end date.
It takes about four minutes and returns a readiness score with the first move for each gap. For the leadership side of the same pattern, see UnitedHealth's new Chief Administrative Officer: what the role inherits.
